Dear Parents,
The following instructions are to help you in the process.
l) The registration form & emergency form are the forms needed to be filled in for all students. You must fill in an emergency for each child. Students are not automatically registered; you must re-register your child for next year. Spaces are limited and based on a first come first served basis. You must also sign saying you read the Parent Handbook. It is available in the "CLASSES" section of our website.
2) If, on the registration form you are also checking off the column for a NEW student, in addition to all other forms, you must fill out a copy of the permanent record card for each new child. Also, for new children entering the program, a copy of the baptism and communion certificate (if applicable) must accompany the forms.
4) For Confirmation and First Holy Communion, there is a sacrament fee of $100.00 to be included with the registration fee.
5) Forms, along with fees, must be received by May 30, 2013.
6) Home-study: If your child is in Grades 3rd -6th and you are choosing to home school your child, a registration form and home school application must be sent to us along with the required registration fees.
7) COTTLE SCHOOL: Parents who are choosing to use the bus for the 2:15pm session must pick-up Release Time Card (orange card) and fill out bus permission form at Religious Education Office. Cards will not be given out at COTTLE, nor can you pick up cards for someone else. Cards will not be mailed.
8) THERE IS NO KINGERGARTEN BEING OFFERED 2013-2014.
